doesnt work

I have used this form many times in many other sites. This time there is a problem. I have done ll the field that i need and went live to test it. I filled in all the fields a if i were a customer and then hit send. It won’t send! The circle/arrow thingy just spins into perpetuity. I check to make sure that the answers were going to the correct email and there is my email filled out ‘mail’ section on the bottom left of my master word press admin.

I am using this on Safari, as I have many times for other sites that I have created.

Nothing. HELP!!!!
