Name appears using code in page but not in sidebar

    The plugin works great (I gave it 5 stars) but I have a question:
    I added it to my sidebar and all it shows is the box where the visitor enters the email address.
    At the same time, I copied the code [smlsubform]and put it at the end of a post. This time it prompts to enter both name and email address.
    How do I make the name field appear in the sidebar as well?
