Looking for a distributor directory plugin without maps or directions

    I am searching for a plugin that will allow the user to utilize a drop down menu of US States and will then display a list of distributors for their state.

    EX: http://www.redkensalon.com/distributor-locator/

    I don’t need maps, driving directions, ratings or any other extraneous information other than the following

    Distributor Name
    Address
    City, State, Zip Code
    Phone Number
    Website

    Is there anything like this available that anyone knows of?
