how to evaluate a checkbox listing (scoring sheet)

    Hi,
    I have done a considerable amount of searching – to no avail…

    I need to offer short answers to questions marked by checkboxes in a list (scoring sheet?) on a normal WP page.

    page 1 = questions + checkboxes
    page 2 = answers to marked questions + email to author

    Depending on which combination of questions is marked, the answers will differ. I would also like the evaluation sent to my email adress.

    Is there such a plugin?
    How can this be done?
