Paypal plugin that can do both set and editable rates

I have been trying to track down a plugin that works with Paypal that does four things:
– can offer flat rate options (donate $10, $20, $30)
– can offer an editable rate (other amount)
– collects contact info.
– allows checkboxes for the user to indicated which fund they’d like their donation to go to.

I’ve found plenty that do one or two, but not all. Even the paid ones don’t seem to have all that I need. Anyone have a clever idea? Thanks!
