    A very good contact form but un fortunately it appears very USA centric.
    Although I can put a non USA phone number in the form does not allow any localized information. e.g. one cannot put in an overseas address as the drop down menu only provides for USA state. Sadly i will have to give this a pass.

    Staton — Just letting you know we did a new release yesterday and included in that release is the functionality you wanted. If you are located outside of the US you can now pick your country and the address fields will adjust accordingly. We’ve also added some other new features like being able to set each of the fields as Required/Optional/Hidden. The Premium plan level has a new template. Our appointment setting feature can also now sync with with Google Calendar.

    Our email service provider integrations are now included at all plan levels including free so if you have an account with Aweber, Constant Contact, MailChimp, or iPost you can have your form submissions go directly to your email provider (zendesk and zoho as well with more on the way).
