After the last update my form colors are wrong!

My fast secure contact form was working great. I believe when I did the last update, the colors and EVERYTHING changed! Now the form doesn’t match my site at all…in fact, it looks like it takes you to a “different” site altogether!
Could someone tell me if anyone has had the same problem or if you can help me get this back to normal…I’d really appreciate it!
I was checking the site today because we are getting VERY few form requests for estimates and I’m pretty sure this is the reason why!
