export to CSV with full of form content

Hi

I created quiz with form on the front.

first you have to fill up form:
– name
– store name
– position (eg. store manager)

next will be quiz with 10 questions.

when I export data to CSV – I ve got quiz results but without form content.

any idea how to fix this problem?
