Contact Form 7 text box not lining up

One of my text boxes is pushed down on Contact form 7 when using FireFox and Chrome.
Works OK in IE.website: http://13009.duvosoftware.com/request_quote/
The Last Name: text box pushes the rest of the text boxes down.
Anyd suggestions?
Also the word “Edit” shows up in FireFox next to the last name text.
How do I get rid of the Edit box on the page?
