Looking for Plugin or Ideas for Post e-mail notification system

Looking for a plugin that would, in theory, alert an email address (stored inside a custom field) after a post has been active (published) for a certain time period (lets say 1 year).

Anyone know of any plugin that has this functionality that I could look at and hack to my specs?
