How let users upload videos to your video gallery

I am looking for a way to let the users (after login) to upload videos from either Youtube or file to a site. Suppose this site has a video galley and the uploaded videos go to this gallery after approval.

I have checked youtube direct lite and tried to incorporate it into my site. I did what the instruction said to do, but it is not working properly. It uploads a webcam video to my youtube account but doesn’t leave a trace of pending or anything else on my site. After refreshing the page, the video goes away as if it never existed. So I quit this attempt.

I am asking the pros here if they know of a better way or a plugin that can do this.
What it needs to do.
1- upload a video from youtube or file
2- pending for moderation
3- if possible goes straight to a video gallery or video page
4- if possible creates a thumbnail list say in sidebar

Seems complicated but I hope there is a way possible to do it.
