add info to fields

There’s probably more than one approach to that. You could put that into the field name.

If you want to do something more advanced (but probably more flexible and cleaner), I would suggest slipping something in using the wpmem_register_form filter (I’m thinking of using php’s str_replace to look for whatever you field name is and replacing that with your extra text).
