Ask Customers Question(s) via radio button

    I want to be able to ask the customer a question or two.

    Ie: How did you find us:
    a) Search Engine
    b) Advertisement
    b) Friend / Contact
    c) Other _______

    Hope that makes sense. I envisage just a simple radio button format. Nice and simple.

    Is there a 3rd party plugin or something you can do in Woocommerce?
