I had that same issue, as we have two business branches in different countries … and it’s not possible in Yoast, so I installed the plugin “Add Meta Tags“. I had to modify it though to disable creating descriptions, so it won’t create duplicate descriptions as Yoast already does it.
Basically while writing a post or page, there will be a field called Full Meta Tags to key in something like :
<link rel=”alternate” hreflang=”ex-default” href=”http://www.astrasuite.com/astrablog/voxativ-best-sound-award-at-rmaf-2012-by-av-showrooms/” />
<link rel=”alternate” hreflang=”en-ID” href=”http://www.lyrics-soundart.com/lyrics-news/voxativ-best-sound-award-at-rmaf-2012-by-av-showrooms/” />

I was trying to set correctly the tag <link rel=”alternate” /> following the guidelines released by google.
Now I’m stuck in front of two problems.
The first one: is it possible to set the x-default property through yoast plugin?
The second one: yoast is not using ISO 639-1 format for the “hreflang” values (It uses _ instead of -). So, is there a way to change that value and make it pursuant to ISO 639-1 standard?
I have a solution: install WPML you can choose alternate tags there!
