Hello @albgen,
I’ve tested your form on the site with Divi theme, and Forminator 1.36.1, after upgrading Forminator to 1.37.1.
I could not replicate the issue, when the “Forminator Styles” option is selected in Appearance >> Design Style: https://prnt.sc/1Da8evrkBPFJ
However, the form style changes similar to your screenshot if I select “None“, and that’s actually what it is meant to do as well, as it stops loading Forminator styles.
Could you please test it by enabling the “Basic” option, and saving the changes. Then please try switching it back to “Forminator Styles“, and save changes again.
If that doesn’t help, please check it again after flushing all caches (Divi theme cache, CDN/server cache, etc.).
If flushing caches doesn’t make any difference, could you check and please let us know, if there are any error messages in the browser console. Here’s how to check them: https://appuals.com/open-browser-console/
