Plugin Help

Hey everyone. This is my first post as I am relatively new to WP.

I am in need of a list style plug-in, I cant seem to find one so maybe it doesn’t exist. I would like to create a list of items in a Page. Some one can click on an item, enter there details and that gets email to me. Then that item is removed form the page.

Sound complicated I know, can this be done with a plugin, if so which ones?
