Company Intranet User Evaluation

Hello,

I am operating an internal business website (specifically an intranet) on the Woffice theme.
The site is specifically for managing multi-locations in retail. It currently has the capabilities of incident reporting, safety report, etc.

I am looking to include one more specific capability and am unsure of how to go about it.
Each store in my network is a user on the site. I want to create a “quiz” where I will visit the store – fill out the quiz, and score each store.
It is a loss prevention audit. I want to give the stores a scored audit based on my visit.

Is there an existing plugin that could assist with this? I have been looking through multiple quiz plugins to see and havent come up with anything.
