Form creation

Hi, please help.
I purchased the Pro plugin and still I am having the hardest time making a form.

These are the issues I am battling:
1. When I move a field and save, the field stays in the original position. I have to try multiple times for it to work.
2. Every time I try to populate a Selectbox -without exception- I get a large numbers of “No title” entries that replace legitimate options. I managed to make this work once as you can see in my form but on any attempt after that, no go.
3. Some text fields get cleared after saving and are blank

Please help, I am loosing my mind just trying to make a simple form.
