Installation

Upload plugin-name.php to the /wp-content/plugins/ directory
Activate the plugin through the ‘Plugins’ menu in WordPress
Place <?php do_shortcode('[SMFA user_id=1]'); ?> in your templates

Go to Users >> All Users. Click on edit ?user_id=2 pick user id from the url user user id for the shortcode.
