Nice concept, but simply version I downloaded simply did not work.
Just checked the version and any extra feature needs the pro-version. How can you really test if everything is limited. Dont bother in downloading.
I have not tested this plugin seriously, because the fact that I had to go to Paypal to find out that the premium version cost 50 bucks has turned me off.
Works great for my project. I don't need the extended version for my project but am going to buy it anyway to support future development.
It fills a gap that Gravity Forms (my usual go-to multi-purpose form plugin) doesn't fill by providing weighted answers that actually work. GF's Quiz is a bit buggy in my experience.
One enhancement suggestion — For my current project I'm loading the quiz in a modal window and routing the user to a URL based on their score. It'd be great to be able to tick an option to open the Route URL in a _blank window. The modal window is too small to fit the destination URL. No biggie, I can get around it but thought it worth mentioning.

This plugin is well coded and has a clean and simple interface.
The reason why I rank it as a 1 out of 5 is because, despite the name, it is a quiz tool not a survey tool. As you enter questions you give a point value to each answer then as the user submits their answers they are given a final score and can be directed to different pages based on their score. This is perfect if you are quizzing.
If, however, you want to survey users this plug-in falls short because you cannot export the user responses. The export that comes with the $50 expanded version just gets you high level details like when the quiz was taken, what their score was, and where they were re-directed to.
THERE IS NO WAY TO SEE THE USER'S SPECIFIC ANSWERS.
This is kinda important for the purposes of a survey. Unfortunately, this shortcoming does not become apparent until after you've paid your $50.
I would change my ranking from a 1 to a 4 if a second export was added so that the specific user response data could be obtained.
I would then change my ranking to a 5 if a couple of other enhancements were added:
1 - Allow an option for the user to enter an "other" answer
2 - Add a ranking question type. E.G. "Put the following items in order of preference."
Summary: If your intent is to conduct surveys, then "WordPress Simple SURVEY" is <u>not</u> the plug-in you want. Instead I would refer you to Google forms. If you wanted a WordPress plugin that keeps the data internal to your local database then I would refer you to "Quiz Master Next" by Frank Corso. This plugin is FREE and while it is also a quiz tool and has some of the same issues, the author is responsive and already committed to added an export of specific user responses.
Newest version is much better and responsive. Great support on extended version, totally worth it.
Normal hosts have mod_deflate enabled. This plugin will NOT work with GZIP / mod_deflate enabled.
After not updating the plugin in a few years, we're back with a re-write, new features, and even more features to come!
The 3.x is a clean break from 2.x and you will have to migrate your old data manually if you choose to upgrade but this break allows us to merge in new features, improve performance, and make your modifications easier.
В конечной форме 2 поля ввести нельзя. Точнее можно, но не видно, что вводишь. Задумка хорошая, но реализация не очень.
Installed correctly, made three test questions and answers, put shortcode on page. Great. Then, tried it out and it starts with the first question, I enter an answer and click next. "ERROR, Name is required." No way to back up to that screen, and no way to go forward. I made the Name and Email fields not required and tried again. This time, I enter a test answer and click next. I am taken to the site home page and kicked out. FAIL.
