Form takes transaction, charges user, and reports the transaction failed
Currently this plugin is completely broken from what I can tell. The form actually charges users but reports the transaction was denied in the UI. This could potentially lead to the user trying to submit several times and being charged each time without knowing it.
Additionally the plugin builds the form with lots of empty paragraph tags and lots of line breaks for no apparent reason. Getting the form to look like what is displayed in the screenshots takes some additional css work.
