Form Maker is a modern and advanced tool for creating WordPress forms easily and fast.
The back end of the plugin works nicely and allows you to create forms easily, although the free version limits you to four fields plus a submit button.
Tables! The forms are output in layers upon layers of nested tables rather than using divs and spans. This makes it practically impossible to do any serious layout customisation and also makes the forms useless on responsive/adaptive sites.
Form fields are given fixed pixel heights and widths, which further limits your ability to customise the layout.
It's two years since the WordPress default theme became responsive, so it'd be really nice if plugins made an effort to output code that would work in the default theme at most screen sizes.
A quick W3C validation check shows errors throughout the forms, mainly because of the tables.
If all you want to do is add a simple form to your site I'm sure it'll do the job, but if you want full control over the layout or to have a responsive/adaptive website this isn't the plugin for you.
I've tried to make this contact form work but I have some major issues with the functionality that disallow me to put my contact form to use:
1. Not Responsive
Fields are given a fixed px length and the form is coded in a table
2. Reply-to Email Address missing Options
While you can make the sender email your "reply-to" email, for some reason you can't do so without sending a copy to the sender
3. Sender Email should be editable or default to your Admin Email instead of your host's server name
I purchased the pro version and was quite happy and pleased with the result at first until the forms started showing some errors (in the editing screen). When I contacted support they asked me to get administrative rights to enter my site to check the problem, I was a bit worried about allowing any one to administer my site especially when the email support from El Dorado are anonymous! I simply asked them to provide me with a name, title and contact phone of the person who will be allowed to help me out, however to my disappointment they replied that they cannot provide personal info about their stuff, which obviously raised my suspicion about their services (I have dealt with numerous online support companies and have never encountered such ridiculous replies!) I suggest to El Dorado guys to be more transparent in their dealings and reveal their "hidden" identity to their clients "who are actually paying them their salaries" otherwise they will start loosing business. Good luck and I hope this advice will not go unnoticed if they are serious about customer service.
I was not able to capture field values from java script using
getElementById("id name").value. Does anyone have the same issue?
Over the last 3-4 weeks I have been trying to use FormMaker, but encountered some difficulty. On every occasion, support responded immediately and remedied my problem. I appreciated their patience in having to repeat their fix to my website after my WordPress site got messed up. They not only came in and fixed my problem again, but provided information on what to do should that same problem occur again. Thanks so much Webdorado!
everything works like it should. all fields are working - very easy to make a form. i choose this form maker due to the graphical charts.

only problem: the moving of the fields has some bugs
Not at all what I expected. Unless you just want a very simple form with only 7 fields including buttons & captcha.
The plugin installed easily and in a general sense is easy to use. I like the ability to easily review submission data.
The bad & Ugly:
The html code generated is some of the poorest I have seen using nested table upon nested table, instead of proper span and div coding.
I have also had major issues with the css not working in my template, so the generated form look nothing like the preview and after spending hours trying to figure it out am on the verge of simply uninstalling it.
The limit of 4 fields (not 5 since the submit button is required and counted in the limit) is simply too restrictive to be able to even test the plugin properly.
I have also ben unable to find a lot of documentation or support to remedy any questions I have (fix css, remove extra 'br' in table which is throwing off the form with lot of extra white space, how to get form in french or better yet bilingual, etc...)
I had issue when reordering fields where it would move around numerous fields (not just the one a clicked on). Thus forcing me to rework multiple fields until I got it right?! Strange bug or something.
Hidden field data does not get sent in e-mail upon submission.
For a simple form it is probably all you'd need, but for advanced forms, and customizations, I think the plugin still needs a lot of maturing!
Installed the plugin and started building only to be told by a pop up that you can only have 5 fields in the free version. So I can't even test what I needed before I have to purchase. Don't bother with this and use another resource. I can't type on here the words which have come out of my mouth. Fewmin.
Form Maker Pro really lets you control nearly every aspect of way the form looks. Very easy to use and very easy to customize the form themes. We had an issue that was caused by our theme and the guys at Web Dorado/Form Maker resolved the issue in a professional and timely manner.
