This contact form plugin in very robust and customizable. It has a lot of options, so it may look really daunting at first, but it's actually easy to use once you've got the hang of it. My go-to contact form.

Perhaps the author can confirm there are no more hidden links in the code..........
Also dont go to his website, you have to sign up to view his plugins....only to nail you with spam emails later.....been there done that.
22.01.2013 - Update - this plugin has been found to be conflicting with a lot of other plugins - more reason not to touch this spammy, badly written plugin.
