Have you tried:
- deactivating all plugins to see if this resolves the problem. If this works, re-activate the plugins one by one until you find the problematic plugin(s).
- switching to the Twenty Ten theme to rule out any theme-specific problems.
- resetting the plugins folder by FTP or PhpMyAdmin. Sometimes, an apparently inactive plugin can still cause problems.

I have tried this and even with all plugins disabled I'm still not getting the WYSIWYG editor.
Any other ideas?
Check that you haven't disabled it in your profile.
The only items unchecked in the profile are:
- Disable the visual editor when writing
- Enable keyboard shortcuts for comment moderation
Will take a good look in my settings as well now but can't remember changing anything there!
Did you try resetting the plugins folder?
May have to try this tomorrow. We are a little wary of tinkering on the database in case we break something (our knowledge of such things is fairly basic).
Will that mean disabling all the plugins again? :-(

This is driving us crazy!!! Tried all options above with no luck.
Still got no WYSIWYG editor icons - see screenshot:

First go here, http://java.com/en/download/testjava.jsp to see if your java is correct and working.
If so, delete the wp-admin and wp-includes folder. DO NOT delete the wp-content folder.
Get a new unzipped version of wordpress and copy the wp-includes and wp-admin folders to the server. Use a ftp program like filezilla.
