I'm currently using WordPress 2.1.3 with WP-ExtremeVideo 2.9.3 and the plugin's button does not appear in the TinyMCE bar in the code or design view. But, when I turn off the visual editor, the quicktag appeared... Anyone know how can I fix this?

Same problem here. It was working and now it seems to have stopped, but I can't work out what's changed. There has been no modification to WordPress or the Plugin, but then the error is apparent independent of web browser platform. Maybe it's a server issue?
