    Is mod_rewrite enabled on your server?

    @Reuben
    Yes, I have tried Twenty ten with zero plugins. Issue persists. I believe it has something to do with a fault in the database. Here is something that I think would help - http://img6.imagebanana.com/img/thuy7coc/Selection_030.png (Its on the category page)

    Do you want me to turn sql queries to save and then get back with those? Sorry I didn't understand completely what debug info you want me to get with the debug bar.

    @esmi
    Posts, Page permalinks work fine, only categories & tags don't. So that proves mod_rewrite is enabled. Right?

    Make a copy of your website locally and test it. I am assuming there is either something wrong with your .htaccess file or the database.

    Seems like your attachment rules are taking over your category rules(maybe because they don't exist in your database).

    Also, try replacing rewrite_rules with the _transient ones but do a database backup first.

    The problem was fixed itself when the site owner deleted .htaccess file and resaved permalinks. Weird but that fixed it.

    I am marking this as resolved!

    Thanks for all the help. :)
