Have you tried switching to the default 2012 theme and disabling all your plugins and then reactivating them one by one to rule out any conflicts?
Ok, I deactivated current theme (ithemes builder) and activated twenty twelve theme, and the submenu items appeared instantly!
So have asked ithemes about this issue also, looks like something to do with their theme.

A friend of mine also said that – Apparently there is a builder update out that will address this issue with the WordPress update.

how did you update builder? I can’t upload it to upgrade in WP, it fails. I can upload it to my cpanel but it is a zip file that won’t open.
