Widget not showing up in dashboard

Having an issue with the dashboard widget not showing up. It’s not showing up in screen options either. I’ve deactivated and reset the plugin several times trying to get it working but nothing works. I have several other sites running the exact same plugins on the exact same theme with zero issues.

Any ideas?
