Which plugin for New Client Registration form?

    I have a new client that needs me to setup a New Client Registration form on his website for him.

    What are some good plugins to check out?

    He would like the new form sent to his email.

    He is a chiropractor, so a form that is editable for specific questions would be preferred.

    I’ve used Fast Secure Contact Form in the past and been happy with that plugin, you can create your own questions/fields with it. Recently used it on a dentist website, so similar to what you are needing. You might also check out Contact Form 7, I’ve used that once recently as well and was also happy with it.
