I've checked on more 1.5.1.1. installs and seemingly the "page(d?)" qyuicktag is not there anymore. At least I cannot see it. However if I insert manually the <!--nextpage--> tag it works.
Is there a reason unknown for me why it was left out?

No idea why it went, but I never update my quicktags.js file so didn't notice. Insructions for it's return are here:
