I have used http://www.oiopublisher.com for a number of years now and never had a problem. Its not free, but it is cheap. OIO Publisher can work as a WP plugin or a standalone. And can serve multiple sites not just the one its installed on.
But it depends on what you are looking for really. I use to use OpenAds, PHPAds or as it is now known OpenX.
It was free and open source... Looks like things have changed a bit (might still be open source, didn't really look to deed):
