Weird Comment Happenings

I searched through several pages of posts before I posted. I apologize if I missed one that already answers this question.

My comments are working fine, with one exception. If the commenter fills out the name/e-mail/URI fields when posting, it doesn’t carry over to the actual post. When I get the notification of the comment, it appears as though someone anonymously commented.

At first I thought folks weren’t entering that information, but when I tested it for myself it did that same thing. Any ideas on what I need to do to fix this?
