video survey

Hi,

I need to make some sort of video survey for the users of a blog. I need to have on one page the videos (embed youtube) in one column and next to them a rating from 1 to 4 (radiobuttons) . Each user can make the survey only once. Users should not see each other ratings and I need to be able to retrieve the results individually for each user to create a personalized analysis later on.

I could not complete the installation of Limesurvey (DB configuration), and I do not find any plugin that explicitly says that “questions” can be video embed.

Can somebody advise me a good plugin?
