   Is seems there are two options.

   Upgrading WordPress Codex
   Upgrading WordPress Extended

   what is the difference between codex and extended?
   How do I determine which is the correct one for my blog?

   These are the extended directions: http://codex.wordpress.org/Upgrading_WordPress_Extended

   Read them and determine which one works for you. Most people use the short directions, since they just work.
