Upgrade from 2.1.2 to 3.3.1

Hello,

I know this is an old error, but I am trying to figure it out. So far, I have not been able to find a solution.

I am upgrading from 2.1.2 to anything the latest. I have tried going to 2.3, 2.7, 3.3.1 directly and step by step.

The problem I am having is that the upgrade does not complete. Very similar to this problem:

It stops, when I restart I get those errors. However, after that completes everything seems normal EXCEPT the category associations are broken. The data is in the tables, I can see wp_term_relationships is populated and the data looks correct. But the backend does not work.

Please help!!!!!
