    Hello,
    Please help, I am unable to add any new images to a site.

    Using the visual editor, the ‘Upload/Insert’ button works and I am able to upload images to the library without any problem, but when I click ‘insert into post’ the ‘add media’ popup overlay box just shows a page with the message ‘Apologies, but the page you requested could not be found.’
    here’s the addrss of the page, obviously it’s an admin page so you can’t actually view it…

    If I try using the html editor and the ‘img’ tag button pretty much the same thing happens, except there’s no overlay ‘add media’ panel so I add the img src tags etc and when I click ‘Update’ I am redirected to the same Not Found page as already mentioned.

    If I try to edit just the text in an existing post with images the same happens, although I seem to be able to edit and update the text on existing pages without a problem. I cannot add new images to existing pages however.
    I’ve also tried inserting images that are hosted elswhere and have the same issue.

    I’ve tried disabling all plugins and reverting to both twenty ten and twenty eleven and still have the same issue.
    I’ve also updated to 3.3.1 and the problem persists.

  • Have you tried:

    – deactivating all plugins to see if this resolves the problem. If this works, re-activate the plugins one by one until you find the problematic plugin(s).

    – switching to the Twenty Eleven theme to rule out any theme-specific problems.

    resetting the plugins folder by FTP or PhpMyAdmin. Sometimes, an apparently inactive plugin can still cause problems.

    Hi Esmi, yes, I’ve certainly tried the first two suggestions. Will try resetting the plugins as well now

    I tried renaming the plugins directory and it didn’t solve the issue.

    Yes to both

    If you continue to have problems despite carrying out all of these steps, try installing & activating
    http://wordpress.org/extend/plugins/use-google-libraries/to force a true browser flush.

    Hi, thanks for the suggestions so far, I’m busy with other work so I might not get a chance try the plugin you suggested until the weekend now, will update when I’ve tried it.

    Sorry it’s been a week or so since I’ve had a chance to look at this again
    OK, tried the ‘use-google-libraries’ plugin suggested and still no joy: Able to post text only posts but as soon as I try to insert an image by ANY method I just get a “Apologies, but the page you requested could not be found” message.
    Would really appeciate any further assistance in solving this issue.

    In another attempt to try fixing this unresolved issue I went to the deshboard and clicked re-install wordpress 3.3.1 and the site is now completely broken, I get a page that says…
    ” Fatal error: Call to undefined function wp() in /home/winscomb/public_html/wp-blog-header.php on line 14 “

    Currently trying to restore the MySQL wp database via cpanel, seems to be taking an awfull long time to restore a 118KB file

    Any help getting the site back would be appreciated

    Managed to restore the site from backups.
    I am still unable to add any images though, as previously described.

    Finally solved, spoke with my host and it was something to do with ‘mod_sec’
