unable to create album or upload images

I was able to create one album, but can’t upload images. i add files and click on start upload. it shows 100%, then a yellow triangle appears on the far right, and it changes to 0%. The message says “Upload complete. 0 images were uploaded.”

I have uninstalled it and deleted it, reinstalled and now I can’t create an album or add files to the one album I have. I was using the Omega theme, changed to 2012 and it still doesn’t work.

This plugin was recommended for the premium theme Emily, but it doesn’t work on that either.
