Unfortunately I don't remember when this started (it's been a long time) so I have no idea what might have caused it, but the Title Toggle checkbox to hide the title on a page is completely missing.
I can hide titles site-wide, but I'd really prefer to just hide them on specific pages, and this plugin no longer does this.
Any suggestions?

It sounds like you have a plugin or theme conflict with the code library we're using for the metabox. You can try disabling your plugins one-by-one to identify the culprit.
If you can identify the plugin that is conflicting with it, I can test and see if there's an issue on my end or with their plugin. If the latter, I'll provide recommendations to that developer on how to fix it.
