Title and Tag displaying twice on search engines (3 posts)

My website is http://www.rezolue.com and I am currently using the theme Adelle by BlueChic. The title of my blog is "Rezolue" and the tag is "passively aggressive". I am having an issue with the title and blog..
When I search my blog, the title and tag will show up twice like this:
Rezolue | passively aggressiveRezolue | passively aggressive
Too Faced Coco Glow Duo | RezolueRezolue
Sephora 2012 Black Friday Haul | RezolueRezolue

This only happened after I installed SEO Yoast, and the issue goes away once I disable it.

    I did this:
    - Deleted title template
    - Checked/Unchecked Force Rewrite option
    - Used "<title><?php wp_title(''); ?></title>" in my header.php
    - Looked for "add_filter('wp_title', 'some_function_name' );" in my functions.php (my theme does not have this code)

Instead it did this: Nothing changed- still having the same issue.

    Please help!

Yeah, issues here too. No one seems to know how to fix it. Will be deleting Yoast Plugin.
