PS Same thing happens if it's in a sidebar widget; it doesn't show up but appears in header.
We (well, I do...) frown on sharing email addresses here in the forum.
But if you scroll down on the plugin page at the bottom you'll see the support email that Michael has thoughtfully provided on the plugin page.

Yes, actually. I just put it in a footer widget so you can see what happens. Site built on Genesis framework. http://theimprintdoctor.com/
I don't see any widget in the footer.
I took it down about an hour ago; client obviously didn't want it up there too long. :(
How about this, create an admin account for the support email address found under support at http://wordpress.org/extend/plugins/testimonials-widget/. Then I can debug when I'm around and needn't worry about upsetting clients.

There's JavaScript troubles.
1. jQuery loaded from Google while the tools and such are from the website. This is a good way to have versioning issues.
2. Uncaught Error: Syntax error, unrecognized expression: div.ostrm_categories_menu(.topmenu,.submenu) div.ostrm_categories_menu_list ul li
When trying out Testimonials Widget, I see what you mean by the box being at the top of the screen. Next, it's not rotating which seems due to jQuery issues.
If you can you fix the jQuery, then I can get back to debugging Testimonials Widget.
