Test works, form fails

I can’t get FSCF to send e-mails. I installed WP-Mail-SMTP and checked the settings using the test function. This works OK. Then I test FSCF with the test function, this also works OK. When I use the form to send a message http://www.kalisti.nl/?page_id=7 I don’t receive it. I’ve allready gone through these steps: http://www.fastsecurecontactform.com/email-does-not-send but it does not seem to help. What can I do?
