TDO Mini Forms Custom Title Permalinks

I am using TDO Mini Forms to create a way for users to be able to submit information about local bars in their area by typing in the Name, Address, City, State and Zip of the one they would like to submit.

The only thing that’s not working for me is trying to pass the values created in the City, State and Name fields into one Title field.

I figure if I can set that as the model I shouldnt have to worry too much about any permalinks overlapping.

I’m kind of a PHP noob so I’m sure there might be something I’m missing but I havent been able to work my way through this one and havent found anyone doing something similar. Would appreciate some guidance. thanks!
