   Hi WordPress!

   I once saw a great survey plugin on this forum, I forgot to bookmark it, and can't find it with Search now. I don't want to pay for Semiologic (it comes with a survey plugin). I am looking for an awesome free survey plugin, that is more than a poll. It's a more complex survey, with many questions, some you can choose just 1 answer, some you can check-all-that-apply, some even have a user-field for "Other" input responses. It would function like the online service, SurveyMonkey (it costs too).

   I *know* I have seen & tested this on a WP blog, somewhere, just can't find it....

   several months later... Anyone seen a good Survey plugin?

   Full on survey, with all the nice options like picking more than 1 answer on a question like your Top 3 favorite colors, or limiting a question to just 1 answer at a time; Fill-in-the-blank for your "Other:" answers; etc.

   Thanks for the ideas Truth. Anyone else got experience with these or others? I'm still looking for better.

   Here's some comments I found on those so far... I've quoted the comments (by others) which reflect some of the features I would want in a true SURVEY plugin. I want the mutliple kinds of questions (such as Choose your 3 Favorite Fruits, with OTHER: Fill in the blank option), and obviously I want it to save the results of each question in a way that I can view the aggregate results, or the individual respondant's answers...

   Last, this site does not appear to have comments? So I don't know if people like this one, or what it does really, it doesn't explain. I might try it on a test-site.

   Holy WordPress! I just finally stumbled across the plugin, that I might have been lookin' for all that time. Anyone tried "PHPmyadahas" WordPress questionaire/survey plugin?

   Amazingly, it offers all this, according to the README (which comes with the download but is not printed on the blog for some reason):

   Question / Answer Type
   There are five types, of which four are categorized by the nature of the answers for those types of questions, and the other is a container/grouping for several sub-questions. Following are descriptions of each of those.

   Text Answer: Single line - will allow the user to specify the answer for the question by a typing a single line of text.

   Text Answer: Multiple lines - will allow the user to specify the answer for the question by a typing a several lines/paragraph of text.

   Selection Answer: Single selection - will allow the user to specify the answer by selecting one of the pre-defined answers. You also have the option to allow the user to specify the answer by typing a single line of text, in case if the answer differs from the pre-defined set of answers.

   Selection Answer: Multiple selection - will allow the user to specify the answer by selecting one/more of the pre-defined answers. You also have the option to allow the user to specify the answer by typing a single line of text, in case if there is any different from the pre-defined set of answers.

   Container Question - will allow you to group a set of questions and display them under this type of question. This may be used when several related questions are used, or to display a comment above a set of questions. A 'Container Question' assumes that questions followed by it are its sub-questions up to the limit specified by the 'No of sub-questions'

   Even though these are described as they are, you can do different things to make it look the way you want it to, so be creative!

   It was released in 2005 and the author's blog appears inactive, but I got the zip. Has anyone tried this plugin with WordPress 2.x?
