Submit Form for data

Hello,
I am looking to create a community-oriented racing log. I would like people to have the ability to register, login, post, and then have a staff member be able to review his post and accept or decline it.
(a) Does this exist?
(b) If not, can someone please create it?

It would be great if there were fields such as name, city, state, country, phone number, general comments, etc.
