Simple Image Widget 2.1 by Chris Vickio not working

I realize I should put this in the plugins area.
I’ve posted there. I uploaded the 2.1 plugin for a simple image widget and put it on my blog: http://leadershipskills.successtelevision.biz
It doesn’t show up.

Can you tell me what to do?
