Simple DB Query Plugin Utility

We are using WP as a company intranet and would like to create an employee cell phone number lookup form to search, for example by: First Name, Last Name, State, Branch Name, etc…. I wonder if 1) a simple DB plugin combined with 2) a form building plugin that can then query that DB would be the best solution to this need.

Any advice that could help direct us would be greatly appreciated. I’ve been nosing through plugins and forums but have yet to find something that I feel could meet the requirement outlined above.
