Do you know if there is a signup widget that includes telephone number (as well as name and email address)? Otherwise, are there any widgets (eg sign up or contact forms) that are easy to configure to do this? Thanks for your help.

Ok, I've solved this using G-Lock Double Opt-in Manager plugin.
