Is this possible, Jeff? I want to show the form, but with sligtly different text and labels on a different page. I tried modifying <?php if (function_exists('user_submitted_posts')) user_submitted_posts(); ?>, but I could not force it.

Can you help me with this?

Yes absolutely, the forms are located in the files submission-form.php or submission-form-classic.php, depending on the plugin settings. There you'll find all of the markup and PHP logic used to render the forms, such as labels, text, and so forth.
