Setting up permanent redirect from old–>new domain?

How do I set up a permanent redirect from an old domain to a new one?
For example http://www.old.com –> http://www.new.com
Is it also recommended I set up redirects for:
old.com –> http://www.new.com and
new.com –> http://www.new.com?My web host is BlueHost if that helps.
Can you help?
