Send Data to Url php page

Hello everyone! I have been researching and looking at examples and can’t seem to find exactly what I need. I am making the form and our IT guy is taking that information from the form and doing something with it.
My question is. He wants me to send the data to a http://www.my-site.com/page.php . I can’t seem to get it to send all that data to the page. I read something about add_filter then about a form action, but none of those seem to be what I need.
Hopefully this is something easy. Any help?
